Using the Parental Accommodation Form

The Parental Accommodation Form is a special purpose form, required to be filled out by the student requesting a leave of absence in anticipation of child birth or adoption. Now that you know where to find this form and how open it, let’s talk about filling it out.

Providing Personal Information

The first few blocks are pretty self-explanatory. Enter your last name, first name, and M Number in the appropriate boxes. If you do not know your M number, you can always find it on MyMichiganTech.

Entering Degree and Advisor Information

Once you have your M Number entered, move on to selecting your degree type. Depending on your degree, there are a few options you will need to select from before moving on to the next step.

Doctor of Philosophy (PhD)

If you are a PhD student, you can select the graduate program from the drop down list.

Master of Science (MS)

If you are an MS student, you will see three degree options appear as shown below. Please indicate whether you have opted for the coursework only track or are preparing and defending a Thesis or Report. After you have done this, please select your graduate program from the drop down list.

Master of Forestry (MFor)

If you are a MFor student, you will see two degree options appear as shown below. Please indicate whether you are a coursework student or are working towards preparing a report.

Master of Geographic Information Science (MGIS)

If you are a MGIS student, you will see only one degree option; coursework.

Master of Engineering (MEng) 

If you are a MEng student, you will only be asked to select your graduate program which will display one option, Non-Departmental.

Master of Business Administration (MBA) 

If you are a MBA student, you will see only one option, coursework, as shown below.

Providing Advisor and Co-advisor details

If you have an Advisor/Co-Advisors please enter their details in this section. If you do not, you may skip to the next section.

Provide the details of Birth or Adoption

Enter the information of anticipated date of birth or adoption and the anticipated start/end dates for your leave.

Providing Additional Requests and Funding details

If applicable, please provide any extension or modifications of the request with regards to deadlines and/or academic expectations as shown below. If you are a supported Graduate Student receiving support from the University, please select the appropriate position from the list below.

Once you have filled in all the information, obtain signatures from the concerned persons and submit it to Graduate School via mail or e-mail at gradschool@mtu.edu.
