The Readmission/Reinstatement form is required for students who have been inactive for academic, medical, or other reasons. You can find the Application for Readmission or Reinstatement form on Forms and Deadlines page or by clicking here. Fill in the form, save the form, and e-mail to firstname.lastname@example.org.
Now that you know where to find this form and how open it, let’s talk about filling it out.
Providing Information on the Type of Admission
Select the appropriate admission that you are enrolled into: Degree/Certificate seeking student or a Non-degree seeking student. When you select the option “Degree or certificate seeking student”, you will have to provide an appropriate reason for the being inactive from the options provided as shown below:
If you are a non-degree seeking student, please check the box next to the statement regarding transfer of credits on the second page of the form as shown below:
Providing Personal Information
Personal Information including Last Name, First Name, and M Number should be entered in the first section of the form.
Entering Degree Information
Once you have your M Number entered, move on to selecting your degree type. Depending on your degree, there are a few options you will need to select from before moving on to the next step.
Doctor of Philosophy (PhD) Options
If you are a PhD student, you can select the graduate program from the drop down list.
Master of Science (MS)
If you are an MS student, you will see three degree options appear as shown below. Please indicate whether you are a Coursework only student or are preparing a Thesis or Report. In addition, select the graduate program from the drop down list.
Master of Forestry (MFor)
If you are a MFor student, you will see options for a Report or Coursework. Please choose the one appropriate for your degree.
Master of Geographic Information Science (MGIS)
If you are a MGIS student, you will see only one degree options, that is coursework, appear as shown below.
Master of Engineering (MEng)
If you are a MEng student, you will not see a degree option. You will only be asked to select your Graduate Program which should be “Non-Departmental.”
Master of Business Administration (MBA)
If you are an MBA student, you will see only one option, coursework, as shown below.
If you are a certificate seeking student, select the type of graduate program certificate from the drop down menu, appear as shown below.
Selecting the Type of Enrollment
Please tell us if you are an online or on-campus student in this section of the form.
Providing Contact Information
In this section, please provide us with your current mailing and off-campus e-mail address as well as your phone number.
Entering Semester Readmission and Degree/Certificate Completion Information
Enter the semester and the year you wish to enroll and also when expect to complete the degree/certificate.
Providing Reasons for Non-continuous Enrollment, Readmission and Advisor Details
Please answer the questions which appear in this section in detail, while keeping in mind the 500 character limit. Once you have done this, you will need to provide us with the name(s) of your advisor/co-advisors.
Entering Information for Conviction or Felony Charges
If you have been convicted of a felony or have any felony charges pending against you, please answer “yes” to this question and provide a detailed statement as an addendum to this form.
