Students may appeal their dismissal as described in the Graduate Appeals of Suspension or Dismissal policy. In the event that the use of this form is necessary, please follow the instructions below to ensure an accurate submission of the form.
Where Do I Find the Form?
The Graduate Appeal Form can be found on the Graduate Appeals of Suspension or Dismissal policy wbepage or by clicking here. Graduate students must submit a completed form to the Graduate School before the dean will consider an appeal request. This form must be submitted to the Graduate School within ten (10) business days of notification via email from the Graduate School of suspension or dismissal.

Now that you know where to find this form and where we can open it, let’s talk about filling it out.
Providing Personal Information
The first section of this form asks for your personal information such as Last Name(surname), First Name(given name), M Number, and Date of submission.
Entering Degree Information
Once you have your M Number entered, move on to selecting your degree type. Depending on your degree, there are a few other prompts you will need to navigate before moving on to the next step.
Doctor of Philosophy (PhD) Options
If you are a PhD student, you can select your graduate program from the drop down list and proceed to the next step.
Master of Science (MS) Options
If you are an MS student, you will see three degree options appear as shown below. Please indicate whether you are a coursework only student or are preparing a thesis or report. In addition, you will be asked to select the graduate program from the drop-down list.
Master of Forestry (MFor) Options
If you are an MFor student, you will see two degree options appear as shown below. Please indicate whether you are a coursework only student or you are preparing a report.
Master of Geographic Information Science (MGIS) Options
If you are an MGIS student, you will see only one degree options, that is coursework, appear as shown below.
Master of Engineering (MEng) Options
If you are an MEng student, you will see only one option, that is Non-departmental, under the graduate program list appear as shown below.
Master of Business Administration (MBA) Options
If you are an MBA student, you will see only one option, that is coursework, appear as shown below.
Providing Enrollment and Contact Information
Please specify the term along with the year that you wish to enroll as shown below. More details on the enrollment policy can be found out by clicking on the “Continuous Enrollment Policy” button at the right side. This section also asks for your on-campus(@mtu.edu) email address and the best phone number to contact you.
Providing Information about the Appeal
In this section, you are expected to provide the Graduate School with information regarding unusual or extenuating circumstances regarding your suspension/dismissal. This information will be provided to the dean and assist them in their determination of your appeal.
In addition, this section also requires you to outline your intended behavior changes and plan of action to ensure success in the future. In the case of missing grades, please describe the plan of action to address the missing grades. You may provide any additional information or documentation that supports your appeal. These supplemental documents should be submitted in PDF form and should be included in the same email in which you submit this form.
After you have successfully completed this form, please attach this and any supplemental documents relating to your appeal to an email and submit it to firstname.lastname@example.org.
