Using the Graduate Student Parental Accommodation Funding Request Form

Funding may be available from the Graduate School to Department Chairs, School Deans, Graduate Program Directors, and Principal Investigators who must hire a temporary replacement for a student who has been granted an excused absence under the Parental Accommodation Policy.

Now that you know where to find this form and how open it, let’s talk about filling it out.

Is This The Right Form For Me?

This form is for departmental use only. Students requesting Parental Accommodation should use the Parental Accommodation form found here. Departments using this form should be sure that the student has submitted the Parental Accommodation form to the Graduate School or this request will not be reviewed.

Providing Student Information

The first few blocks are pretty self-explanatory; enter the last name, first name, and M Number of the student requesting leave in the appropriate boxes.

Entering Degree and Program Information

Once you have entered the identifying information of the student requesting leave, you will need to provide information regarding their degree.

Doctor of Philosophy (PhD) Options

If they are a PhD student, you can select the graduate program from the drop down list.

Master of Science (MS) Options

If they are an MS student, you will see three degree options appear as shown below. Please indicate whether they are pursuing the Coursework, Thesis, or Report option. In addition, select the graduate program from the drop down list.

Master of Forestry (MFor) Options

If they are a MFor student, you will see two degree options appear as shown below. Please indicate whether the student is pursuing the coursework or report option.

Master of Geographic Information Science (MGIS) Options

If they are a MGIS student, you will only see Coursework appear for the degree option as shown below.

Master of Engineering (MEng) Options

If they are a MEng student, there will be no degree option. Instead, you will need to choose Non-Departmental as the Graduate Program before proceeding.

Master of Business Administration (MBA) Options

If they are an MBA student, there should only be a Coursework option in this section.

Providing details of Student Position and Funding

Please select the position held by the student and the source of funding.

Entering Information on Temporary Replacement

Please enter information regarding the temporary replacement. This information should include length of employment, hourly rate, and the number of hours worked per week. The “Total Amount” field will auto calculate once the other fields are filled. Please remember that you should also attach supporting documents to justify this budget request when submitting this form.

For this section, please enter your details including your full name and your relation to the student requesting leave.

Once this form is completed it should be submitted to the Graduate School. Keep in mind that this form must be submitted at least three weeks in advance of the proposed parental accommodation period. Forms can be submitted via campus mail( Graduate School, 4th Floor Admin Building) or via email to gradschool@mtu.edu.
