Waiver Instructions

WAIVER INSTRUCTIONS

  • Download the Approval Waiver – Form #3
  • Complete the form. Include: Name, M#, Course CRN, Course Subject # Numbers, Semester/Year
    NOTE: If you are completing a Time Conflict Waiver, make sure you list course information for BOTH courses. 
  • Check the box that applies to your waiver need. 
  • Send the form to the instructor via email. In your email, make a formal request. Be professional and detailed. Request they sign your form if they agree.
    NOTE: You must obtain signatures from BOTH instructors if it is a Time Conflict Waiver.
  • If they agree and sign, send the completed and signed form to the correct person: 
    –Send to bmeadvisor@mtu.edu if you are a BME Student and the course(s) at hand are BE courses.
    –Send to schedule@mtu.edu for processing if you are not a BME student OR if the courses is NOT a BE course. 
  • Once the waiver is processed, you may the course(s) in question.
